In the mere five years since our inception, OHN’s team approach has achieved optimal care in every major patient health risk category we measure. With compliance rates of 79% for hypertension control, 82% for chronic kidney disease screening and 56% for vascular care screening, we’re establishing a proven record of success and a very positive trend for the future.
One way we’re achieving these goals is through the Ochsner Accountable Care Network (OACN), a collaboration of over 2,700 providers working together to provide and improve the quality and value of care being provided to over 31,000 Medicare beneficiaries across Louisiana and Mississippi.
In 2019, OACN reduced their expected cost of care by more than $23 million, representing a 53% increase in savings (nearly $800) per patient, and a savings increase of 81% over 2018. OACN seamlessly coordinates care to increase efficiency and keep patients healthy at home.
Our patients interact with us through:
- our patient portal,
- virtual visits on their phones and
- digital medicine programs to proactively manage chronic conditions in a way that encourages greater involvement in their own healthcare.
These more frequent and convenient touch points have allowed OACN to keep patients out of the hospital and home with their families.
Efforts that contributed to OACN’s 2019 MSSP results include:
- Improved preventive care delivery by increasing annual wellness visits, decreasing unnecessary emergency room (ER) visits and increasing life-saving screenings for common cancers and other health issues, including depression.
- Enhanced access to primary and urgent care services and focus on communication.
- Improved collaboration with patients’ care teams through Epic, a best-in-class electronic health record.
- Utilization of algorithms and advanced analytics that help identify and predict health risks in patients.
- Sharing of robust health data, tools and technology to support clinicians in the care of our most vulnerable patients who suffer from chronic conditions.
